Your Role With Us:At Salem Health, Pharmacy Residencies give Pharmacology postgraduates advanced clinical training, to develop skills and competence in providing pharmaceutical care to a variety of patients in various hospital settings, under the supervision of an experienced preceptor.Core Functions:Focus Supervises technicians who assist the pharmacist in the practice of pharmacy. Assumes responsibility for accurate and timely drug distribution. Monitors ongoing therapy for continued appropriateness of drug, dose, route, and duration. Makes modifications of therapy (based on internal guidelines) or recommends modifications of therapy to providers when necessary based on age, renal function, height/weight, medical condition, etc. Provides pharmacokinetic dosing and monitoring of aminoglycosides and vancomycin. Documents treatment plan and rationale when appropriate in the progress notes. Maintains continuing competence in practice areas through participation in continuing education programs and self study as required by the State of Oregon and consistent with clinical practice focus. Initiates appropriate pharmaceutical plan of care to address holistic, age-specific needs and reflect the patient's changing condition.
